What are HCP?

Home Care Packages(HCPs)  are government-funded programsand structured into four levels of care. These levels are designed to reflectthe varying degrees of care and support needed by older individuals to remainsafe, independent and well-supported in their own homes. At Aman Care Supportat Home, we deliver services aligned with these levels, while ensuring care isculturally and religiously sensitive, personalised and respectful of eachclient’s individual needs.”

Basic Care
Level 1
For individuals who require minimal assistance with daily living.
Occasional help with activities such as showering, dressing and preparing light meals
Basic cleaning or householdtasks
Support to stay safe and connected
Low-Level Care
Level 2
For those needing a little more support to remain independent.
Regular help with personal care and mobility
Light domestic support (cleaning, laundry, etc.)
Assistance with medications and health monitoring
Social support and transport to appointments
Intermediate Care
Level 3
For individuals with moderate care needs.
Frequent assistance with personal care and household tasks
Help with managing medications and mobility

How to Get Started?

7 Steps in the below formatso it is easy to understand

Check Eligibility

The consumer must be over 65years old or over 50 years old and identify as Aboriginal or Torres StraitIslander, and have an assessed need for support with activities of daily living, health, or mobility.

Contact My Age Care

Call My Aged Care on 1800200 422 to start the process of getting a home care package. They will ask for basic information about the consumer’s situation, including age, health andliving arrangements

Assessment for Eligibility and Care Needs

An Aged Care Assessment Team(ACAT) will contact the consumer to arrange an assessment of their care needs.This assessment will determine the level of care required and whether a homecare package is appropriate.

Approval for a
Home Care Package

If the consumer is approved fora home care package, they will be placed in the national priority queue and notified when a package becomes available. If not approved, other options for care will be discussed.

Choosing a Home Care Provider and Package Level

Once the consumer receives an approval letter, they can start looking for a home care provider. Aman Care isa Home Care provider and we will help the consumer choose the most appropriate package level and develop a care plan

Signing a Home CareAgreement & StartingServices

The consumer and Aman Care will sign a home care agreement, which sets out the terms and conditions of the home care package. This agreement can be reviewed and updated as the consumer’s needs change. Once the home care agreement is signed, the consumer can start receiving home care services from Aman Care. Aman Care will work with the consumer to deliver the agreed care plan.

Reviewing and Adjusting the Care Plan

An Aged Care Assessment Team(ACAT) will contact the consumer to arrange an assessment of their care needs.This assessment will determine the level of care required and whether a homecare package is appropriate.
